FOR A HEALTHIER OTTAWA COUNTY
Our objective is to obtain information and feedback from Ottawa County residents, health care professionals, and key community leaders in various industries and capacities about a wide range of health and health care topics to gauge the overall health climate of Ottawa County and address the identified needs.
COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA)
The CHNA looks at the people’s health of Ottawa County. It is a method to find key health problems and resources, a tool to develop strategies to address health needs, and an opportunity for community engagement and collaboration.
Community Health Improvement Plan (CHIP)
The CHIP is a plan that focuses on the greatest health needs in Ottawa County. Community members, including people from health care and human service agencies, identified three priority health areas based on data from the Ottawa County CHNA.
